No, multi-center corporations cannot apply as a single entity. However, their individual centers or organizations are eligible to apply independently if they meet the other requirements.
The survey eligibility criteria are calculated based on two cycles of Medicare/Medicaid certification surveys. The cycles are weighted differently, with the most recent cycle having the most impact. Applicants must meet all three criteria, including having no Immediate Jeopardy or Substandard Quality of Care deficiencies, a weighted two-cycle survey score at or below 60 points, or at least three stars on the Survey component of the Five-Star System.
ICFs/IID must have a count of Conditions of Participation Deficiencies on their most recent standard surveys below their State Average for the past two fiscal years (October 2021 to September 2023) to be eligible for the Gold Award. Special consideration is provided for applicants exceeding the state average by less than 1 point.
Yes, they can apply for either the entire organization or a distinct part of it. The distinct part must be a separately licensed level of care serving a specific market segment in a clearly defined location.
If an applicant doesn't meet the survey eligibility criteria, they can still apply and receive valuable feedback on their application, but they will not be eligible to receive the Gold Award. They can, however, maintain active status in the program.
